Registration Information
All teams and players participating in the Puma Generation Cup must provide the following documents as the final step in their tournament registration.
-State Association/League Approved Roster - ALL TEAMS NEED TO SUBMIT
-IYSA Tournament Roster - ALL TEAMS NEED TO SUBMIT
-Risk Management Certification Form - ALL TEAMS NEED TO SUBMIT
-Player Passes - ALL TEAMS NEED TO SUBMIT - All players must have a league issued player pass to participate. All players must be passed by the same governing body. Will be verified upon check-in at Puma Generations Cup
-IYSA Medical Release & Liability Waiver - REQUIRED FOR ALL NON-IYSA TEAMS (this includes out of state USYS teams). All players/parents need to complete and submit. All waiver forms must have been completed, signed, and dated no more than 12 months prior to the last day of the tournament (5/21/23).
-Form NT - REQUIRED FOR ALL US CLUB TEAMS (NISL)
-Guest Player forms - All players that are rostered with another club need to submit guest player forms. Forms can be found on the Tournament Center (HTG Sports) HERE. To access the necessary documents you will need to login.
-Travel Permit by state association - All out-of-state teams need to complete and submit. Forms can be found on the Tournament Center (HTG Sports) HERE. To access the necessary documents you will need to login.
When uploading forms to GotSport, please be sure that they are all separate files. When uploading waivers, please do so in one file.
Team Coaches/Managers should have a copy of all of these documents with them at all times when on site and the ability to present them when requested.
